Warren Buffett famously said, 'Volatility is far from synonymous with risk.' It's only natural to consider a company's balance sheet when you examine how risky it is, since debt is often involved when a business collapses. We note that Cytosorbents Corporation (NASDAQ:CTSO) does have debt on its balance sheet. But is this debt a concern to shareholders?

What Risk Does Debt Bring?

Debt assists a business until the business has trouble paying it off, either with new capital or with free cash flow. Part and parcel of capitalism is the process of 'creative destruction' where failed businesses are mercilessly liquidated by their bankers. While that is not too common, we often do see indebted companies permanently diluting shareholders because lenders force them to raise capital at a distressed price. Having said that, the most common situation is where a company manages its debt reasonably well - and to its own advantage. When we think about a company's use of debt, we first look at cash and debt together.

What Is Cytosorbents's Net Debt?

As you can see below, Cytosorbents had US$14.2m of debt, at March 2025, which is about the same as the year before. You can click the chart for greater detail. However, it does have US$11.6m in cash offsetting this, leading to net debt of about US$2.60m.

How Strong Is Cytosorbents' Balance Sheet?

Zooming in on the latest balance sheet data, we can see that Cytosorbents had liabilities of US$9.74m due within 12 months and liabilities of US$26.5m due beyond that. Offsetting these obligations, it had cash of US$11.6m as well as receivables valued at US$7.67m due within 12 months. So its liabilities outweigh the sum of its cash and (near-term) receivables by US$17.0m.

While this might seem like a lot, it is not so bad since Cytosorbents has a market capitalization of US$55.9m, and so it could probably strengthen its balance sheet by raising capital if it needed to. However, it is still worthwhile taking a close look at its ability to pay off debt. There's no doubt that we learn most about debt from the balance sheet. But ultimately the future profitability of the business will decide if Cytosorbents can strengthen its balance sheet over time. Over 12 months, Cytosorbents reported revenue of US$35m, which is a gain of 15%, although it did not report any earnings before interest and tax. We usually like to see faster growth from unprofitable companies, but each to their own.

Caveat Emptor

Over the last twelve months Cytosorbents produced an earnings before interest and tax (EBIT) loss. Indeed, it lost a very considerable US$16m at the EBIT level. When we look at that and recall the liabilities on its balance sheet, relative to cash, it seems unwise to us for the company to have any debt. So we think its balance sheet is a little strained, though not beyond repair. Another cause for caution is that is bled US$14m in negative free cash flow over the last twelve months. So suffice it to say we consider the stock very risky. When analysing debt levels, the balance sheet is the obvious place to start. However, not all investment risk resides within the balance sheet - far from it. We've identified 5 warning signs with Cytosorbents (at least 1 which shouldn't be ignored) , and understanding them should be part of your investment process.
