At 18 months, patients treated with the RNS System experienced a 77% median reduction in GTC seizures compared with baseline, and 40% of patients experienced GTC seizure freedom at that timepoint. The study also demonstrated rapid reductions in other generalized seizure types, including absence and myoclonic seizures. Additionally, more than 90% of patients reported improvement on the Patient Global Impression of Change, and 80% of physicians reported improvement on the Clinical Global Impression of Improvement at 18 months. Further, per request of the FDA during the review process, the Company provided 24-month data from its NAUTILUS study to supplement the aforementioned 18-month data. Patients continued to improve with stimulation; among the evaluable participants with completed 24-month post-implant follow-up, those who received stimulation for 23 of 24 months post-implant achieved a median percent reduction in GTC seizures of 100% compared with baseline, underscoring the potential for responsive neuromodulation to deliver meaningful and durable benefit in a patient population with no approved neuromodulation or surgical treatment options. As previously communicated, NeuroPace’s management guidance does not include any contribution from the expanded IGE indication. Importantly, however, the data did show a clinically meaningful and highly statistically significant response in the primary effectiveness endpoint within a subgroup of patients with lower baseline frequency of generalized tonic-clonic seizures which represented the majority of trial participants. While the study is not powered to show effectiveness in subgroups, this analysis suggests that there is a therapeutic and clinically meaningful benefit of the RNS System in a substantial subpopulation. Additionally, within the entire trial population, clinically relevant data, including median percent seizure reduction, responder rates and improvement in seizure free days showed numerically robust and clinically meaningful improvements over the first year of treatment and continued in those who have progressed to the second year. Notably, in a more challenging epilepsy syndrome which was studied in an ethical and novel study design for medical devices, preliminary data at one-year illustrates the value of the RNS System technology to individualize patient treatment through programming over time. While a distinct study design, these metrics were favorable and higher than rates observed in the Company’s randomized controlled trial in focal epilepsy that led to FDA approval. Following receipt of the communication, NeuroPace met with FDA to discuss the Agency’s feedback and the additional information needed to place the PMA Supplement in approvable form. At the recommendation of FDA and based on that discussion, the Company will be submitting a Submission Issue Request (“SIR”) to further discuss its planned approach with FDA and intends to amend its submission with supplemental information, including additional data and context regarding the patient populations evaluated in the NAUTILUS clinical trial. The PMA Supplement is supported by data from NeuroPace’s NAUTILUS study, the first randomized controlled trial of neuromodulation in drug-resistant IGE. The 18-month NAUTILUS results were published in Epilepsia in June 2026 in an article titled “Responsive stimulation of the thalamus for idiopathic generalized epilepsy: Results of the randomized controlled NAUTILUS trial through 18 months.” The publication provides Level 1 evidence supporting responsive thalamic stimulation as a potential treatment approach for patients with drug-resistant IGE and provides an important peer-reviewed foundation for the dataset reviewed by FDA. At 18 months, patients treated with the RNS System experienced a 77% median reduction in GTC seizures compared with baseline, and 40% of patients experienced GTC seizure freedom at that timepoint. The study also demonstrated rapid reductions in other generalized seizure types, including absence and myoclonic seizures. Additionally, more than 90% of patients reported improvement on the Patient Global Impression of Change, and 80% of physicians reported improvement on the Clinical Global Impression of Improvement at 18 months. Further, per request of the FDA during the review process, the Company provided 24-month data from its NAUTILUS study to supplement the aforementioned 18-month data. Patients continued to improve with stimulation; among the evaluable participants with completed 24-month post-implant follow-up, those who received stimulation for 23 of 24 months post-implant achieved a median percent reduction in GTC seizures of 100% compared with baseline, underscoring the potential for responsive neuromodulation to deliver meaningful and durable benefit in a patient population with no approved neuromodulation or surgical treatment options. As previously communicated, NeuroPace’s management guidance does not include any contribution from the expanded IGE indication. Management will provide full second quarter 2026 financial results and any updates to full year outlook on its second quarter 2026 earnings conference call scheduled for August 11, 2026.

The study successfully met its primary 12-week post-implant safety endpoint, demonstrating a low rate of serious adverse events related to the device and implant procedure, consistent with the established safety profile of the RNS System. The study did not reach statistical significance for the primary effectiveness endpoint in the overall study population, which was to show a longer time to a second generalized tonic-clonic seizure in the active stimulation group compared to the sham stimulation group. Importantly, however, the data did show a clinically meaningful and highly statistically significant response in the primary effectiveness endpoint within a subgroup of patients with lower baseline frequency of generalized tonic-clonic seizures which represented the majority of trial participants. While the study is not powered to show effectiveness in subgroups, this analysis suggests that there is a therapeutic and clinically meaningful benefit of the RNS System in a substantial subpopulation. Additionally, within the entire trial population, clinically relevant data, including median percent seizure reduction, responder rates and improvement in seizure free days showed numerically robust and clinically meaningful improvements over the first year of treatment and continued in those who have progressed to the second year. Notably, in a more challenging epilepsy syndrome which was studied in an ethical and novel study design for medical devices, preliminary data at one-year illustrates the value of the RNS System technology to individualize patient treatment through programming over time. While a distinct study design, these metrics were favorable and higher than rates observed in the Company’s randomized controlled trial in focal epilepsy that led to FDA approval. In the patients that have completed more than 12 months in the study, preliminary metrics showed continued clinical improvement over time, which aligns with prior findings from the RNS System in the focal epilepsy population, including the FDA-reviewed three-year data from the Company’s Post-Approval Study. The NAUTILUS Study is a prospective, multicenter, randomized, single-blind, sham-stimulation controlled study that enrolled 100 participants, with 87 undergoing implantation of the RNS Neurostimulator and Leads across 23 epilepsy centers within the US. Patients had drug-resistant idiopathic generalized epilepsy with a high burden of generalized tonic-clonic seizures, the most severe and dangerous seizure type. NeuroPace received FDA Breakthrough Device Designation status in 2021 for the potential use of its RNS System to treat IGE. Patients in the NAUTILUS trial continue to participate in the study through the completion of two years after the device implant, with prespecified collection of safety and effectiveness data. The Company plans to submit the full dataset to FDA and for peer-reviewed publication. The Company will engage with the FDA to discuss regulatory pathways based on the data. These discussions may include the possibility of utilizing the overall median seizure reduction data across the full study population, as well as pursuing a more targeted indication focused on patients with lower baseline seizure frequency who may represent a majority of patients with IGE.

The pre-specified primary effectiveness endpoint is at three years of treatment. Announces Early Completion of Patient Enrollment in NAUTILUS Pivotal StudyNeuroPace, Inc. announced the completion of the patient enrollment goal in its NAUTILUS pivotal study for the treatment of idiopathic generalized epilepsy (IGE) more than one quarter ahead of schedule. NAUTILUS is the first and only pivotal clinical study to evaluate neuromodulation therapy in idiopathic generalized epilepsy, or IGE, and, if successful, RNS could secure the first device-based indication for generalized epilepsy. 40% of drug-refractory epilepsy patients are diagnosed with generalized epilepsy, and NeuroPace believes that this early patient enrollment milestone is reflective of the excitement amongst patients and clinicians in potentially addressing this significant unmet need with the RNS System. Patients with generalized epilepsy do not need Phase II invasive monitoring and would be able to proceed to RNS System therapy faster, through a noninvasive diagnostic process that could be performed both within and outside of the Comprehensive Epilepsy Center setting. These efforts are part of NeuroPace’s broader focus of expanding access to RNS System therapy to aid in addressing the treatment gap for the 1.2 million drug refractory epilepsy patients in the United States. IGE is the second most common type of epilepsy, after focal onset epilepsy, where seizures originate from various parts of the brain at once. It is typically diagnosed in childhood or adolescence and often results in life-long seizures. Patients with IGE often have seizures that may cause loss of consciousness and commonly experience injuries from seizure-related falls. Further consequences from uncontrolled seizures include poor cognitive outcomes, depression, decreased social interaction with peers, increased seizure frequency, and sudden unexplained death in epilepsy (SUDEP). The NAUTILUS Study received an Investigational Device Exemption (IDE) approval from the U.S. Food and Drug Administration (FDA) and is a prospective, multicenter, single-blind randomized, sham stimulation controlled pivotal study designed to demonstrate that the RNS System is safe and effective as an adjunctive therapy for patients aged 12 and older with drug-resistant IGE. NeuroPace received FDA Breakthrough Device Designation status in 2021 for the potential use of its RNS System to treat IGE. The trial requires evaluation of a primary safety endpoint and an effectiveness evaluation 12-months post-implant. If the study achieves its primary endpoints, NeuroPace plans to submit a PMA Supplement and also intends to publish findings in a peer-reviewed medical journal.
