Cancer is a highly dynamic process, and cells are known to diversify over time due to treatment selection pressure and the accumulation of genetic changes. This leads to the emergence and expansion of new subclones with distinct characteristics and varying abilities to survive and proliferate. This process is crucial because it drives cancer development, progression, resistance to therapy, and relapse and can help in understanding how to develop effective cancer treatments. This publication provides evidence of genetic diversity in CTC clusters in breast cancer patient samples and preclinical mouse models. It includes the finding that some mutations were exclusive to specific cells within CTC clusters that could therefore be missed by a tissue biopsy. The research in mouse models reports a higher prevalence of CTC clusters in high-complexity tumours, with large CTC clusters associated with higher genetic diversity. The genetic diversity reported in this publication points to CTC clusters as key contributors to genetic diversity in metastasis. The authors believe that genetic diversity within CTC clusters enhances their metastatic capability by increasing therapy resistance opportunities, evasion of immune cell attack, as well as adaptability and survival at a metastatic site. CTC clusters are therefore important targets to stop the spread of cancer given these are up to 100 times more metastatic than individual CTCs, with metastatic spread responsible for more than 90% of cancer related deaths. A quantitative HER2 test, that objectively measures HER2 expression levels, enables the identification of patients who are HER2-high, intermediate or low and therefore suitable for treatment with HER2-antibody drug conjugates (ADCs). The poster reports on a study in 16 patients with metastatic breast cancer, in which the Parsortix system was used to isolate and harvest CTCs and CTC-clusters prior to automated imaging and classification as HER2-high, intermediate, low or negative. Using the Parsortix system, CTCs were identified in 100% of patients and their HER2 status classified. The workflow presented in this poster has the potential to be used to assess the current HER2 status of these patients and identify those whose HER2 status has changed making them eligible for the treatment. HER2 is a major therapeutic target and an accurate and real-time assessment of HER2 status could be used to monitor CTCs over time to better guide treatment and identify patients with any detectable HER2 expression, or changes to HER2 expression, who could benefit from treatment with ADCs. Analytical verification demonstrated that the assay successfully produced linear data with high analytical sensitivity1 and specificity2 for epithelial, mesenchymal, blood lineage and PD-L1 markers. PD-L1 allows cancer cells to evade the host immune response when upregulated, and assessment of PD-L1 status in tumor tissue can indicate if immunotherapy has the potential to be an effective treatment. However, lung tissue biopsies can be challenging and expensive and are not always successful, plus PD-L1 status in tumour tissue can become outdated during tumour evolution and may not be representative of metastatic sites. ANGLE has developed a liquid biopsy assay that allows for minimally invasive, longitudinal monitoring of PD-L1 status In CTCs harvested using the Parsortix® system. The assay identified CTCs in 91% (29/32) of metastatic lung cancer patients assessed as part of an ongoing longitudinal study, presenting with a range of phenotypes. Researchers used the Parsortix® system to isolate and harvest CTCs from 30 patients with EGFR-mutated NSCLC that were undergoing treatment with osimertinib (TAGRISSO: AstraZeneca). Osimertinib is an EGFR inhibitor with annual revenues of c. US$6.7 billion, growing at c. 16% per annum. Both CTCs and ctDNA were analysed using a range of molecular techniques which enabled the identification of mutations, methylation, gene expression and amplification. The study provides further evidence that complementary information can be obtained through dual analysis of CTC-DNA and ctDNA. Using the dual analysis technique, at disease progression an increased number of patients were identified with the EGFR T790M resistance mutation and DNA methylation, compared to analysis of either analyte alone. Dual analysis of these liquid biopsy analytes enables longitudinal monitoring of patients throughout clinical trials and may enable early identification of non-responders and identify patients becoming resistant to treatment before disease progression. Additional analysis of CTCs identified druggable molecular markers, such as HER2, PD-L1, PIM-1 and AXL, at study baseline and at disease progression, providing information on the potential benefits of combined targeted treatment, and possible suitable therapies for those patients whose cancer progresses due to resistance to osimertinib. The study was conducted as an offshoot of the European multi-centre GANNET53 Phase II clinical trial(NCT02012192) funded by the EU 7th Framework Programme (Grant agreement ID: 602602). The study investigated the efficacy of ganetespib in combination with paclitaxel vs. paclitaxel alone. Patients were enrolled across 12 clinical centres in Germany, Belgium, France, and Austria. The translational study, which the research team believes is the largest study of CTCs in ovarian cancer (in terms of number of patient samples analysed), was conducted by researchers at the Medical University of Vienna, Austria. The study analysed a total of 474 blood samples collected from 123 PROC patients at baseline (i.e. prior to first administration of study drugs), and at multiple timepoints over a period of two and a half years during treatment until disease progression. A panel of 27 gene transcripts (RNA) was analysed on Parsortix-harvested CTCs using standard laboratory qPCR analysis. The authors identified two CTC-associated markers with potential prognostic value. ERCC1, a key gene of the DNA damage response pathway, was associated with an increased risk of disease progression and worse outcomes, whereas the presence of ESR1, a gene encoding oestrogen receptor alpha (ERa), was associated with a reduced progression risk. The presence of ESR1 transcripts together with concurrent absence of ERCC1 transcripts in CTC-enriched samples at baseline and during treatment cycles was found to be predictive of improved progression free survival (PFS). Whereas the presence of ERCCI transcripts and the absence of ESR1 transcripts at baseline and during treatment cycles indicated a 12.77× greater likelihood (odds ratio) of cancer progression. The analysis of these biomarkers has the potential to provide an early indication of PFS ahead of clinical trial results and suggests that CTC characterisation may be a valuable tool for pharma drug trials in the future. In addition, the authors conclude that molecular characterisation of CTCs before and during treatment has the potential to be a useful tool to monitor ovarian cancer patients and may provide further insights into the biology of this difficult to treat disease. The assay is provided as a service from ANGLE's Onc-ADaPT™ GCP-compliant laboratories and conducted by an expert team with over 10 years' experience in CTC analysis. CTCs captured and harvested using ANGLE's Parsortix® technology are subsequently enumerated and characterised with the Portrait Flex assay. Samples are analysed using immunofluorescence staining for epithelial, mesenchymal, blood lineage and nuclear markers, with the opportunity to include an additional biomarker tailored to customer needs. Examples of clinically relevant CTC biomarkers that have been researched using Parsortix technology and published in peer-reviewed publications include HER2, PIK3CA, PD-L1, EGFR, BRAF, AR-V7 among others. The Portrait Flex assay has an analytical sensitivity of >93%, and an analytical specificity of >95% for both epithelial and mesenchymal markers. Data from the analysis of clinical samples from 16 metastatic breast cancer patients identified CTCs in 81% of patients, with 38.5% of the CTC-positive patients having =1 CTC with high HER2 levels highlighting the capability to assess current HER2 status in breast cancer patients. Of the patients with CTCs, approximately half showed a mesenchymal only phenotype, while the others showed a mixed phenotype, highlighting the importance of Parsortix epitope-independent isolation of CTCs. Combining the use of the Parsortix technology and the Portrait Flex assay allows for testing that is specific to customer needs and can enhance their clinical trial evaluations. The study utilised the Parsortix®? system to investigate the genetic diversity of CTC clusters. Understanding the genetic diversity of CTC cluster is of clinical importance because these cells are the precursors of progressive disease and metastasis. Cancer is a highly dynamic process, and cells are known to diversify over time due to treatment selection pressure and the accumulation of genetic changes. This leads to the emergence and expansion of new subclones with distinct characteristics and varying abilities to survive and proliferate. This process is crucial because it drives cancer development, progression, resistance to therapy, and relapse and can help in understanding how to develop effective cancer treatments. This publication provides evidence of genetic diversity in CTC clusters in breast cancer patient samples and preclinical mouse models. It includes the finding that some mutations were exclusive to specific cells within CTC clusters that could therefore be missed by a tissue biopsy. The research in mouse models reports a higher prevalence of CTC clusters in high-complexity tumours, with large CTC clusters associated with higher genetic diversity. The genetic diversity reported in this publication points to CTC clusters as key contributors to genetic diversity in metastasis. The authors believe that genetic diversity within CTC clusters enhances their metastatic capability by increasing therapy resistance opportunities, evasion of immune cell attack, as well as adaptability and survival at a metastatic site. CTC clusters are therefore important targets to stop the spread of cancer given these are up to 100 times more metastatic than individual CTCs, with metastatic spread responsible for more than 90% of cancer related deaths. The authors conclude that CTC clusters carry cells from different tumour clones, and that the assessment of CTCs and CTC clusters using the Parsortix system may provide insights into genetic diversity that overcomes the spatial and temporal limitations associated with traditional tissue biopsy.

The findings, published by Weill Cornell Medicine, corroborate ANGLE's own findings from its HER2 assay development programme, and provide a semi-automated solution which may help to accelerate adoption of CTC-based HER2 tests. HER2 is a protein that is associated with many cancers, and HER2 tumour marker testing is used to determine if targeted therapy will be effective. A quantitative HER2 test, that objectively measures HER2 expression levels, enables the identification of patients who are HER2-high, intermediate or low and therefore suitable for treatment with HER2-antibody drug conjugates (ADCs). The poster reports on a study in 16 patients with metastatic breast cancer, in which the Parsortix system was used to isolate and harvest CTCs and CTC-clusters prior to automated imaging and classification as HER2-high, intermediate, low or negative. Using the Parsortix system, CTCs were identified in 100% of patients and their HER2 status classified. The workflow presented in this poster has the potential to be used to assess the current HER2 status of these patients and identify those whose HER2 status has changed making them eligible for the treatment. HER2 is a major therapeutic target and an accurate and real-time assessment of HER2 status could be used to monitor CTCs over time to better guide treatment and identify patients with any detectable HER2 expression, or changes to HER2 expression, who could benefit from treatment with ADCs. HER2 status is known to change in up to 38% of breast cancer patients. As such real-time quantitative HER2 assessment is crucial for both effective treatment and to support patient stratification for clinical trials and insight into dynamic changes in HER2 status which may correlate to therapeutic response.

Analytical verification demonstrated that the assay successfully produced linear data with high analytical sensitivity1 and specificity2 for epithelial, mesenchymal, blood lineage and PD-L1 markers. PD-L1 allows cancer cells to evade the host immune response when upregulated, and assessment of PD-L1 status in tumor tissue can indicate if immunotherapy has the potential to be an effective treatment. However, lung tissue biopsies can be challenging and expensive and are not always successful, plus PD-L1 status in tumour tissue can become outdated during tumour evolution and may not be representative of metastatic sites. ANGLE has developed a liquid biopsy assay that allows for minimally invasive, longitudinal monitoring of PD-L1 status In CTCs harvested using the Parsortix® system. The assay identified CTCs in 91% (29/32) of metastatic lung cancer patients assessed as part of an ongoing longitudinal study, presenting with a range of phenotypes. PD-L1 positive CTCs were observed in 72% of patients with a PD-L1 positive tissue biopsy and in 27% of patients with a PD -L1 negative tissue biopsy, demonstrating the potential for improved assessment of tumour heterogeneity through analysis of PD-L1 statusin CTCs. A patient case study revealed that CTC PD-L1 status varied between blood draws, showing discordance with the initial tumour biopsy at multiple time points. The results presented in this study demonstrate the potential for the development of CTC-based dynamic PD-L1 testing to advance personalised treatment for metastatic lung cancer patients.

Researchers used the Parsortix® system to isolate and harvest CTCs from 30 patients with EGFR-mutated NSCLC that were undergoing treatment with osimertinib (TAGRISSO: AstraZeneca). Osimertinib is an EGFR inhibitor with annual revenues of c. US$6.7 billion, growing at c. 16% per annum. Both CTCs and ctDNA were analysed using a range of molecular techniques which enabled the identification of mutations, methylation, gene expression and amplification. The study provides further evidence that complementary information can be obtained through dual analysis of CTC-DNA and ctDNA. Using the dual analysis technique, at disease progression an increased number of patients were identified with the EGFR T790M resistance mutation and DNA methylation, compared to analysis of either analyte alone. Dual analysis of these liquid biopsy analytes enables longitudinal monitoring of patients throughout clinical trials and may enable early identification of non-responders and identify patients becoming resistant to treatment before disease progression. Additional analysis of CTCs identified druggable molecular markers, such as HER2, PD-L1, PIM-1 and AXL, at study baseline and at disease progression, providing information on the potential benefits of combined targeted treatment, and possible suitable therapies for those patients whose cancer progresses due to resistance to osimertinib. This adds to the body of evidence supporting ANGLE's large pharma strategy, highlighting the potential to aid in the stratification of patients that may benefit from alternative/combinatorial therapies being investigated in clinical trials.

The study recruited 76 MBC patients whose blood samples were processed using the Parsortix PC1 systems. This highlights the limitations of EpCAM-based CTC enrichment technologies and further underpins the rationale for the use of ANGLE's Parsortix system which harvests CTCs based on the size and deformability of the cells. The data generated by this study supported ANGLE's FDA De Novo request for the classification of the Parsortix PC 1 system as a Class II medical device, for use in MBC patients to capture and harvest CTCs for subsequent, user-validated, downstream analyses. This was granted by the FDA in 2022.

공시 • May 17ANGLE plc Announces Publication of Study Using the Parsortix SystemANGLE plc announced the publication of a study using the Parsortix system to identify markers present in CTCs harvested from metastatic mutant-p53 platinum-resistant ovarian cancer (PROC) patients. The study was conducted as an offshoot of the European multi-centre GANNET53 Phase II clinical trial(NCT02012192) funded by the EU 7th Framework Programme (Grant agreement ID: 602602). The study investigated the efficacy of ganetespib in combination with paclitaxel vs. paclitaxel alone. Patients were enrolled across 12 clinical centres in Germany, Belgium, France, and Austria. The translational study, which the research team believes is the largest study of CTCs in ovarian cancer (in terms of number of patient samples analysed), was conducted by researchers at the Medical University of Vienna, Austria. The study analysed a total of 474 blood samples collected from 123 PROC patients at baseline (i.e. prior to first administration of study drugs), and at multiple timepoints over a period of two and a half years during treatment until disease progression. A panel of 27 gene transcripts (RNA) was analysed on Parsortix-harvested CTCs using standard laboratory qPCR analysis. The authors identified two CTC-associated markers with potential prognostic value. ERCC1, a key gene of the DNA damage response pathway, was associated with an increased risk of disease progression and worse outcomes, whereas the presence of ESR1, a gene encoding oestrogen receptor alpha (ERa), was associated with a reduced progression risk. The presence of ESR1 transcripts together with concurrent absence of ERCC1 transcripts in CTC-enriched samples at baseline and during treatment cycles was found to be predictive of improved progression free survival (PFS). Whereas the presence of ERCCI transcripts and the absence of ESR1 transcripts at baseline and during treatment cycles indicated a 12.77× greater likelihood (odds ratio) of cancer progression. The analysis of these biomarkers has the potential to provide an early indication of PFS ahead of clinical trial results and suggests that CTC characterisation may be a valuable tool for pharma drug trials in the future. In addition, the authors conclude that molecular characterisation of CTCs before and during treatment has the potential to be a useful tool to monitor ovarian cancer patients and may provide further insights into the biology of this difficult to treat disease. According to clinicaltrials.gov there are 1,024 active interventional studies enrolling more than 825,000 participants in ovarian cancer.

The researchers used the Parsortix system to harvest CTCs for analysis and concluded that "… gene expression in CTCs could be pivotal to prescribing more targeted treatment based on the needs of the patient." In this study, pre-clinical mouse models of melanoma brain metastasis were split into four treatment groups and two cancer drugs were tested individually and together vs. placebo. Blood samples were analysed using the Parsortix system to harvest CTCs. The authors found that using the two drugs together was the most effective therapeutic regime, reducing tumour burden, metastasis and CTC count. This research provides further evidence of the capability of the Parsortix system to be used for the harvesting of CTCs for analysis in pre-clinical mouse models. The global market for preclinical clinical research organizations exceeded a valuation of USD 5 billion in 2022 and is expected to achieve a 7.5% CAGR over the next 10 years owing to increasing R&D expenditure on early-stage development combined with the growing number of drugs in preclinical development.
