Announces FDA Approval of the WRAPSody Cell-Impermeable EndoprosthesisMerit Medical Systems, Inc. announced that the WRAPSODY® Cell-Impermeable Endoprosthesis has received premarket approval from the US Food and Drug Administration (FDA). With this approval, Merit can begin commercialization of the device in the USA in 2025. WRAPSODY is designed to extend long-term vessel patency in dialysis patients. Many patients undergoing dialysis rely on a vascular access site created in the arm called an arteriovenous (AV) fistula (surgically joining an artery and vein) or an AV graft (a soft tube placed in the arm). The maintenance of adequate blood flow through these sites is crucial for patient survival. Over time, life-threatening access site complications can arise, including stenosis (narrowing of the vasculature) and thrombosis (blood clot formation). Each year in the United States, 687,000 procedures are performed to maintain dialysis access; nearly 100,000 of these procedures involve placement of a stent (a flexible woven metal tube, often with a covering) to preserve blood flow. While covered stents have improved long-term patency, stenosis is still common in patients receiving hemodialysis treatment. A key contributor to stenosis is the accumulation of tissue within the polytetrafluoroethylene (PTFE) layer of these devices. To address this challenge, in 2010, Merit engineers collaborated with Bart Dolmatch, MD, FSIR, Interventional Radiologist at Palo Alto Medical Foundation in Palo Alto, California, to develop the WRAPSODY Cell-Impermeable Endoprosthesis. The WRAPSODY Cell-Impermeable Endoprosthesis consists of a proprietary covering that features a nitinol stent frame enveloped by an expandable PTFE outer layer, an inner-luminal layer of novel “spun” PTFE designed to reduce platelet and fibrin formation,1 and a middle cell-impermeable layer designed to prevent transgraft tissue migration or accumulation.1 The nitinol frame provides enhanced radial force, compression resistance, and softened ends to help the device conform to vessels, withstand physiological compression, and reduce stress on vessel walls. Results from the WRAPSODY WAVE pivotal trial demonstrated that AV fistula and AV graft patients receiving treatment with the WRAPSODY device for dialysis outflow lesions achieved a target lesion primary patency of 89.8% and 82.0%, respectively, at six months. The WRAPSODY CIA is intended to add to Merit's growing portfolio of clinical evidence supporting the WRAPSODY CIE. Real-world outcomes associated with the use of the WRAPSODY Cie are also being evaluated in the WRAP Global registry, also being conducted by Merit, which was designed to enroll up to 500 patients outside of North America. Radial access offers many well-documented benefits compared to the femoral approach,1 but there remain opportunities to improve patient outcomes, as radial artery spasm (narrowing) and occlusion (blockage) can still occur. Radial artery spasm is reported in up to 20% of patients,2 and approximately 4.6% will have a radial artery inclusion, both of which can lead to patient discomfort and other complications.4,5 Sheath features, such as a hydrophilic coating, a low profile, and tapered transitions have been shown to help reduce the risks of radial arterial spasm and occ Lusion as well as improve patient comfort. Long-term access to blood vessels (vascular access) is critical for patients undergoing hemodialysis. However, stenosis, a narrowing of blood vessels, is a common complication that can have life-threatening consequences if not resolved. Intervention options for patients affected by stenosis have been limited. The WRAPSODY CIE was developed to help physicians effectively treat patients on hemodialysis who experience vascular access stenosis. In 2010, Merit set out to understand how and why previous devices failed to provide lasting benefits to patients. It became evident that in the previous generation of covered stents/stent grafts a substantial cause of device failure and, ultimately, loss of vascular access, was the presence of tissue within the polytetrafluoroethylene (PTFE) layer. With this understanding and key clinical insight from Bart Dolmatch, MD, FSIR, Interventional Radiologist at The Palo Alto Medical Foundation in Palo Alto, California, engineers at Merit designed the WRAPSODY CIE. The tri-layer design of the device is unlike any other treatment option currently available. The three layers of the WRAPSODY CIE consist of: (1) an expandable nitinol stent frame fully enveloped by an outer layer of expanded PTFE; (2) a cell-impermeable middle layer that prevents tissue accumulation and migration; and (3) an innermost internal layer that consists of novel "spun" PTFE to reduce fibrin and thrombus formation. Other unique features of the WRAPSODY Cie include optimized radial force, high compression resistance, and softened ends, all of which help the device conform to vessels, reduce stress, and improve dialysis effectiveness.

Updates ASGE Clinical Practice Guideline Includes TIF 2.0 and cTIF for Management of GERDMerit Medical Systems, Inc. announced that the American Society for Gastrointestinal Endoscopy (ASGE) updated its guideline on the diagnosis and management of gastroesophageal reflux disease (GERD) to include Transoral Incisionless Fundoplication (TIF 2.0) and TIF 2.0 Consecutive Transoral Incisionless Fundoplication (cTIF) procedures as evidence-based approaches to care. GERD occurs when stomach acid flows into the esophagus, causing irritation of the tissue lining. Uncomfortable symptoms can include heartburn, upper stomach or chest pain, trouble swallowing, and chronic cough. A common chronic condition, GERD affects about 20% of people in the United States.1 Left untreated, GERD can lead to other serious complications over time. A minimally invasive therapy, the TIF 2.0 procedure reconstructs the gastroesophageal valve and restores its function as a reflux barrier. TIF 2.0 can also be combined consecutively with a surgical hiatal hernia repair, in a procedure referred to as cTIF. To diagnose and manage GERD, the ASGE developed an evidence-based guideline using the Grading of Recommendations Assessment, Development, and Evaluation framework. Updated from the 2014 ASGE guideline, the revised guideline addresses the role endoscopic anti-reflux therapy plays in the management and treatment of GERD, including TIF 2.0 and cTIF procedures. To help treat this patient population, Merit Medical offers the EsophyX® Z+ Device for use during TIF 2.0 and cTIF procedures. Designed to restore anatomy and reconstruct the gastroesophageal valve, the EsophyX Z+ Device helps provide relief of GERD symptoms and reduce acid reflux that can lead to long-term complications and risk. The EsophyX Z+ Device with SerosaFuse Fasteners and Accessories is indicated for use in transoral tissue approximation, full thickness plication and ligation in the GI tract and is indicated for the treatment of symptomatic chronic gastroesophageal reflux disease in patients who require and respond to pharmacological therapy. It is also indicated to narrow the junction and reduce hiatal hernia =2cm in size in patients with symptomatic chronic gastroesophageal reflux disease. Patients with hiatal hernias larger than 2cm may be included, when a laparoscopic hiatal hernia repair reduces the hernia to 2cm or less.

Today, minimally invasive treatment options offer alternatives to surgery or delayed treatment after watchful waiting. Merit proposes to enroll at least 55 premature infants with hsPDA at up to ten U.S. study sites. To be eligible, patients must weigh between 600 g and 2,500 g, which are considered "extremely low birth weight" and "low birth weight," respectively. Merit intends to evaluate safety and efficacy through six months as part of the premarket approval application. The PREEMIE study is an Investigational Device Exemption (IDE) study. The Bloom Micro Occluder System is not currently approved to treat PDA.

With this approval, Merit can begin commercialization of the device in the USA in 2025. WRAPSODY is designed to extend long-term vessel patency in dialysis patients. Many patients undergoing dialysis rely on a vascular access site created in the arm called an arteriovenous (AV) fistula (surgically joining an artery and vein) or an AV graft (a soft tube placed in the arm). The maintenance of adequate blood flow through these sites is crucial for patient survival. Over time, life-threatening access site complications can arise, including stenosis (narrowing of the vasculature) and thrombosis (blood clot formation). Each year in the United States, 687,000 procedures are performed to maintain dialysis access; nearly 100,000 of these procedures involve placement of a stent (a flexible woven metal tube, often with a covering) to preserve blood flow. While covered stents have improved long-term patency, stenosis is still common in patients receiving hemodialysis treatment. A key contributor to stenosis is the accumulation of tissue within the polytetrafluoroethylene (PTFE) layer of these devices. To address this challenge, in 2010, Merit engineers collaborated with Bart Dolmatch, MD, FSIR, Interventional Radiologist at Palo Alto Medical Foundation in Palo Alto, California, to develop the WRAPSODY Cell-Impermeable Endoprosthesis. The WRAPSODY Cell-Impermeable Endoprosthesis consists of a proprietary covering that features a nitinol stent frame enveloped by an expandable PTFE outer layer, an inner-luminal layer of novel “spun” PTFE designed to reduce platelet and fibrin formation,1 and a middle cell-impermeable layer designed to prevent transgraft tissue migration or accumulation.1 The nitinol frame provides enhanced radial force, compression resistance, and softened ends to help the device conform to vessels, withstand physiological compression, and reduce stress on vessel walls. Results from the WRAPSODY WAVE pivotal trial demonstrated that AV fistula and AV graft patients receiving treatment with the WRAPSODY device for dialysis outflow lesions achieved a target lesion primary patency of 89.8% and 82.0%, respectively, at six months. The primary patency of the entire access circuit at six months in patients with an AV fistula and AV graft were 72.6% and 68.8%, respectively.
